Hormones govern nearly every aspect of a woman's health — mood, energy, sleep, metabolism, cognition, libido, bone density, cardiovascular health, and more. When they're in balance, you feel like yourself. When they're not, the effects can be profound and wide-ranging.

Yet hormonal imbalances in women are among the most frequently dismissed complaints in medicine. "It's just stress." "It's just aging." "Your labs are normal." These responses leave millions of women suffering from conditions that are both real and treatable.

This guide covers the major hormonal shifts women experience across the lifespan, what symptoms they produce, and what can be done about them.

The Key Hormones in Women's Health

Understanding women's hormonal health starts with knowing the major players:

Estrogen — the primary female sex hormone, produced mainly by the ovaries. Estrogen regulates the menstrual cycle, supports bone density, maintains cardiovascular health, influences mood and cognition, and affects skin, hair, and libido. There are three main forms: estradiol (E2, the most potent), estrone (E1, dominant after menopause), and estriol (E3, produced mainly during pregnancy).

Progesterone — produced by the corpus luteum after ovulation. Progesterone balances estrogen, supports sleep, has calming effects on the nervous system, and is essential for maintaining pregnancy. It declines significantly in perimenopause.

Testosterone — yes, women produce testosterone too, in smaller amounts than men. It's important for libido, energy, muscle mass, bone density, and mood. Testosterone declines with age and is often overlooked in women's hormone evaluations.

DHEA — an adrenal hormone that serves as a precursor to both estrogen and testosterone. Declines with age and chronic stress.

Thyroid hormones (T3 and T4) — not sex hormones, but profoundly important for women's health. Women are 5–8 times more likely than men to develop thyroid disorders, and thyroid dysfunction mimics many hormonal symptoms.

Cortisol — the stress hormone from the adrenal glands. Chronic cortisol elevation disrupts the entire hormonal system through multiple mechanisms.

Perimenopause: The Transition That Starts Earlier Than You Think

Perimenopause — the transition to menopause — typically begins in the early-to-mid 40s, though it can start in the late 30s. It's defined by fluctuating and eventually declining estrogen and progesterone levels, and it ends 12 months after the final menstrual period (menopause).

The average duration of perimenopause is 4–8 years. That's a long time to feel like your body is working against you.

Symptoms of Perimenopause

The symptom picture of perimenopause is broader than most women expect:

Menstrual changes:

  • Irregular periods — cycles may shorten, lengthen, or become unpredictable
  • Heavier or lighter flow
  • Skipped periods

Vasomotor symptoms:

  • Hot flashes — sudden waves of heat, often with sweating and flushing
  • Night sweats — disrupting sleep and contributing to fatigue
  • Cold chills following hot flashes

Sleep disturbances:

  • Difficulty falling asleep
  • Waking in the night (often with hot flashes)
  • Non-restorative sleep

Mood and cognitive symptoms:

  • Irritability, mood swings, emotional reactivity
  • Anxiety — often new-onset or worsening
  • Depression
  • Brain fog, memory lapses, difficulty concentrating

Physical symptoms:

  • Vaginal dryness and discomfort
  • Decreased libido
  • Joint pain and stiffness
  • Weight gain, particularly abdominal
  • Hair thinning
  • Skin changes

Many women are surprised to learn that anxiety, brain fog, and mood changes are hormonal symptoms — not just psychological responses to life stress. Estrogen has significant effects on serotonin, dopamine, and GABA systems in the brain. As it fluctuates and declines, so does mood stability.

Estrogen Dominance: When the Balance Shifts

Estrogen dominance refers to a state where estrogen is high relative to progesterone — either because estrogen is elevated, progesterone is low, or both. It's extremely common in perimenopause (when progesterone often declines before estrogen does) and in younger women with certain patterns of hormonal imbalance.

Symptoms of estrogen dominance include:

  • Heavy, painful periods
  • PMS — bloating, breast tenderness, mood changes before menstruation
  • Fibroids and endometriosis
  • Weight gain, particularly in the hips and thighs
  • Fatigue
  • Anxiety and mood instability
  • Thyroid dysfunction (estrogen affects thyroid hormone binding)

Addressing estrogen dominance may involve supporting progesterone levels, improving estrogen metabolism (through diet, liver support, and specific nutrients), and reducing environmental estrogen exposure (from plastics, pesticides, and certain personal care products).

Thyroid Dysfunction: The Great Imitator

Thyroid disorders are far more common in women than men, and they mimic virtually every other hormonal condition. Hypothyroidism (underactive thyroid) produces:

  • Fatigue and sluggishness
  • Weight gain despite normal diet
  • Cold intolerance
  • Brain fog and memory problems
  • Depression
  • Hair loss
  • Constipation
  • Dry skin

The most common cause of hypothyroidism is Hashimoto's thyroiditis — an autoimmune condition where the immune system attacks the thyroid gland. Hashimoto's often goes undiagnosed for years because TSH can remain normal while antibodies are actively destroying thyroid tissue.

A complete thyroid evaluation includes TSH, free T4, free T3, reverse T3, and thyroid antibodies (TPO and TgAb). Standard care often checks only TSH — missing the full picture.

Bioidentical Hormone Replacement Therapy (BHRT)

For women with significant perimenopausal or menopausal symptoms, hormone replacement therapy is often the most effective intervention available. BHRT uses hormones that are chemically identical to those produced by the human body — typically estradiol, progesterone, and sometimes testosterone.

The evidence on BHRT has evolved significantly since the 2002 Women's Health Initiative study (which used synthetic, non-bioidentical hormones) raised concerns about breast cancer and cardiovascular risk. More recent research on bioidentical hormones — particularly transdermal estradiol and micronized progesterone — shows a much more favorable safety profile.

For most healthy women under 60 who are within 10 years of menopause onset, the benefits of BHRT — relief of symptoms, protection of bone density, cardiovascular benefits, improved quality of life — outweigh the risks.

  • Brain fog and cognitive symptoms
  • Vaginal dryness and sexual dysfunction
  • Bone density preservation

Delivery Methods

BHRT is available in several forms:

  • Transdermal patches or gels — estradiol applied to the skin; avoids first-pass liver metabolism
  • Vaginal estradiol — for local symptoms (dryness, discomfort) with minimal systemic absorption
  • Oral micronized progesterone (Prometrium) — the bioidentical form of progesterone; has calming, sleep-supportive effects
  • Testosterone cream or pellets — for women with low libido, fatigue, or mood symptoms related to low testosterone
